Hospital in the Home (HITH) program provides short term acute hospital substitution for patients in their place of residence. This care is delivered by medical staff and nursing staff.

Currently we are seeking a Clinical Nurse Specialist to join the HITH Clinical Service Planning and Population Health unit. The role of the Clinical Nurse Specialist (CNS) is to work autonomously within the Hospital in The Home (HITH) service to deliver quality patient care at an advanced practice level. The CNS is primarily responsible for the provision of a consultation service, and to lead and deliver services within the community and clinics, within their area of expertise.
