Position: Clinical Care Manager (Community Registered Nurse) - Full Time
Location: Melbourne Northern Suburbs and Surrounding Areas
Responsibilities: As a Clinical Care Manager, you will play a vital role in providing exceptional care to our clients in the comfort of their homes. Your responsibilities will include, but are not limited to:
- Offering guidance and care during the transition from hospital to home support.
- Providing short-term post-surgery care, chronic and acute wound care management, palliative care, continence care, catheter care, medication administration, and general wellness checks
- Delivering person-centered care to aged care and disability clients in the community
- Ensuring a safe environment within clients' homes, utilizing clinical skills to enhance quality of life and independence.
- Assessing, planning, and implementing client care within your scope
- Considering physical, social, and psychological requirements and limitations
- Upholding clinical quality compliance standards
- Collaborating with clients' families, allied health professionals, GPs, specialists, and hospital personnel
- Fully equipped nursing kit provided.

Qualifications- Current registration as a Registered Nurse (RN) with AHPRA
- Proven experience in clinical care management
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team.
- Passion for providing outstanding care to clients.
- Clinical Expertise: Proven experience in providing direct client care and ensuring safety.
- Government Funding: Good understanding of the government funding environment for aged care and NDIS (not compulsory but preferable).
- Hospital Discharge: Good understanding of the hospital discharge environment.
- Health Assessments: Good understanding of health assessments and developing care plans to meet clients' care needs.
- Multitasking and Time Management: Ability to multitask and manage time effectively.
- Communication: Excellent verbal and written communication skills.
- Computer Skills: Intermediate to advanced computer skills, including familiarity with CRM systems. #J-18808-Ljbffr
