Our mission is to empower individuals to live fulfilling lives by providing access to essential services and programs. As we continue to expand our reach, we are seeking a dynamic and experienced Head of Support Coordination to lead our team in delivering exceptional support services.

Position overview

The Head of Support Coordination will play a pivotal role in leading the coordination, access, and support services and programs. The successful candidate will be responsible for developing and implementing strategies to enhance service delivery, managing a team of Access & Support Coordinators, building relationships with stakeholders, ensuring compliance with relevant legislation and standards, and contributing to sector best practices.


Key Responsibilities

  • Lead and manage a team of Support Coordinators
  • Oversee the development, implementation and evaluation of Support Coordination Programs
  • Build and maintain strong relationships with participants, families, community organisations and funding bodies
  • Ensure adherence to legislation, standards and guidelines, while developing and monitoring quality assurance processes for improvement
  • Contribute insights and recommendations to the strategic planning process

About you

  • Extensive management experience in the disability or community services sector with a focus on Support Coordination or Case Management
  • Comprehensive understanding of NDIS and related complexities
  • Proven ability in team management and strategic planning
  • Passion for promoting the rights and well-being of people with disabilities
  • Understanding of government grants and R&D grants
  • Tertiary Qualifications in Social Work, Psychology, Community Services or related field (desirable)
