About the Role
As a Care Manager, you'll play a pivotal role in supporting the Facility Manager to ensure the highest standards of clinical care for our residents. Your responsibilities will include:
- Clinical Supervision: Lead and supervise clinical staff to ensure best practices in evidence-based care.
- Leadership and Person-Centred Care: Champion person-centred care and inspire your team to deliver exceptional service.
- Documentation and Reporting: Accurately document and report on clinical and service outcomes to maintain high standards.
- Infection Control: Oversee and enforce stringent infection control principles and procedures.
- Communication: Facilitate effective communication between staff and residents/representatives to foster a collaborative care environment.
- Collaborative Relationships: Maintain strong working relationships with care staff, medical practitioners, and allied health professionals, ensuring optimal use of the Aged Care Funding Instrument (AN-ACC).
About You
To thrive in this role, you will need:
- AHPRA Registration: Registered Nurse with current AHPRA registration.
- Experience in Aged Care: Proven experience in aged care or the ability to demonstrate relevant experience.
- Leadership Skills: Previous leadership roles or demonstrated leadership skills.
- Clinical Competence: Ability to assess, plan, deliver, and evaluate care for residents.
