Additional attributes we are looking for include:

  • Prior experience in assertive outreach, youth work, case management, mental health, justice or AOD
  • The ability to build rapport with people and think outside-the-box to achieve positive outcomes
  • The ability to contribute positively within a team and work autonomously to manage your case load
  • Demonstrated capacity to work flexibly, possess the ability to manage competing demands, and able to effectively set boundaries
  • Qualifications in psychology, mental health, social work or other relevant fields an advantage

About the Role
The Assertive Outreach Service (AOS) is a program focused on delivering a targeted, time-limited-service response to a small number of clients with complex functional needs, who experience significant service gaps and present an unacceptable risk of harm to others and/or self.

The AOS program will deliver assertive outreach and case management services to clients referred by the Department of Families and Fairness (DFFH) across the Bendigo and Loddon Regions.
Within the program, the Assertive Outreach Support Facilitator will support people to overcome barriers to accessing services. This is provided flexibility in the place of residence or community, to build capacity for people with complex needs and improve their engagement with targeted interventions and services.
Working under the leadership of the Practice Leader, the Assertive Outreach Worker will deliver practical support allowing participants to obtain their desired goals.


The role entails:

  • Provide assertive outreach and psychosocial support to young people and adults who are experiencing service gaps
  • Work with participants, clinical teams and other stakeholders (e.g. family) to access and
  • engage with a range of supports
  • Develop, monitor and track recovery plans whilst encouraging engagement and reducing risk
  • Complete risk assessments and analysis
  • Engage participants within the justice system to build and establish rapport and reduce risk


This role works on a hybrid system, including working from home and assertive outreach support to participants across Melbourne, primarily the Bendigo and Loddon Region.
Program hours are typically 8am to 8pm with potential for weekend work. This is a contract position ending 30/06/2025. Opportunity for contract extension is subject to program funding.
It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.
