About the Role

In this role you will ensure a person centred, timely, high quality and cost-effective service is coordinated and provided to meet participant’s needs and scheme objectives.

The successful candidate will be able to handle complex and sensitive conversations, understand how disability impacts on daily living and identify when the individual would benefit from early intervention.

Duties

It’s an exciting time at icare where no two days are the same; as a Rehabilitation Coordinatoryou will be:

  • Using a person-centered approach to assist in identifying and reviewing the needs of participants/workers across all phases of recovery and community participation.
  • Facilitating support to enhance each participants’/workers’ capacity to achieve their desired goals, as identified in their My Plan.
  • Working collaboratively with internal specialist services, external case managers, health professionals and other key stakeholders to coordinate the most effective options to meet the treatment, care and support needs of the participant/worker.
  • Applying critical reasoning and thinking within a decision-making framework in regard to approving recommendations and funding for reasonable and necessary/reasonably necessary treatment, care and support within agreed functional delegations.
  • Managing service approvals and payments in accordance with agreed needs, assessments and plans.
  • Adhering to the KPI’s that monitor and measure key areas of the role
  • Keeping abreast of and understanding the legislative framework and Guidelines and how these contribute and impact the decision-making framework.
  • Working collaboratively with all teams across Care, in the design and implementation of internal quality assurance, management of disputes and complaints and continuous improvement systems to drive quality and consistency in decision making

Skills & Experience

  • Strong practical experience in identifying, reviewing and coordinating services for people and families experiencing serious illness or injuries or disability.
  • A sound understanding of the needs and perspectives of people with disabilities.
  • Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
  • Excellent communication skills – ability to build rapport and communicate complex information and create and nurture effective working relationships with all stakeholders.
  • Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
  • Ability to travel within NSW and possibly interstate depending on the Region.
  • A current driver’s licence.
  • Compliance with Pre-employment checks (including Working With Children Check)
  • Up to date with current Public Health Order requirements
