Vr General Practitioner For Condobolin, Nsw Region

Condobolin, NSW region Ideally based in Dubbo, NSW or Condobolin, NSW if don't mind the travel every 2 days / week Areas to cover: Dubbo – 13 residents Condobolin – 46 residents 16 hours per week (2 days – Mondays and Tuesdays) We are working with a company that are looking for Vocationally or Generally registered GP to work on a refer and request basis for an Aged Care GP Program.
The Doctor will be using a combination of visits or telehealth to service the areas.
Items will be bulk billed on refer and request provider Number.

We do this by offering a Collaborative Care Model with a team of doctors, nurses, geriatrician, carers and allied health professionals supporting the health care needs of residents.

· Improved access to doctors and specialists through a combination of face to face consultations and virtual care · Increased frequency of monitoring through regular routine consultations avoiding preventable hospitalisations and improving health outcomes · Improve coordination of care to ensure residents receive the right care in the right place at the right time · Increase the efficiency and effectiveness of health services for residents, particularly those at risk · Access to affordable health care through a bulk billed service model · Improved continuation of care and clinical handover through integration of health systems and records · Seamless and simple booking system · Secure data access and video conferencing · Allows closer involvement of family members and carers with consultations with facility staff and health care professionals · Stronger collaboration between clinicians in primary care and the care facility · This model addresses several of the Recommendations from the Royal Commission into Aged Care Eligibility: Doctors must have Vocational or General Registration' Doctor must have full Australian work rights Doctors must have experience in ED and or General Practice.
Doctors that are restricted by the moratorium are able to apply.
Must be able to do an onsite visit once every 4 weeks (once a month).
Telehealth every other week.
