Established in 1991, Danila Dilba Health Services (DDHS) is a not-for-profit, Aboriginal community-controlled organisation that is committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) region in the Northern Territory.
We are focused on acute care, immunisation, chronic disease management, women's health, men's health, child and maternal health and sexual health.
We also provide a range of community programs in areas including health promotion, mental health, social and emotional wellbeing, and alcohol, tobacco, and other drugs.
The Dietitian provides services to clients eligible under the Commonwealth Home Support Program (CHSP). The role is underpinned by the CHSP wellness approach, working with clients to maximise functional independence and ability to participate and remain living in the community. The Dietitian works in a multi-disciplinary team to provide high quality, culturally safe services that meet the needs of Danila Dilba clients, with a focus on chronic diseases.
WHAT YOU WILL DO Provide contemporary and culturally appropriate dietetic services for CHSP eligible clients including assessment, treatment, planning, and evaluation.Maintain quality client medical records. Work collaboratively with the Allied Health team to provide holistic allied health services that meet the needs of DDHS clients.Supervise Indigenous Allied Health Assistants and support skill development.Manage caseload and client bookings to ensure timely, coordinated service delivery.Contribute to public health nutrition activities including the development of local, culturally responsive education resources and provision of group dietetic services.Contribute to reporting for DDHS and funding bodies, taking responsibility for accurate content and reporting deadlines.Contribute to effective clinical governance arrangements and service improvement processes that ensure high levels of quality and safety and evidence-based practice.SELECTION CRITERIA Understanding of and commitment to Aboriginal community control and self-determination.Tertiary qualification in nutrition or dietetics that enables eligibility for Accredited Practising Dietitian status.Demonstrated knowledge and experience providing high quality dietetic services to older clients in a community setting.Proven ability to work independently and as part of a multidisciplinary team.Demonstrated understanding of the primary health care context, principles of chronic disease prevention and management and goal-directed care planning.Understanding of the issues and barriers that affect the wellbeing of Aboriginal and Torres Strait Islander people.Well-developed interpersonal skills with a strong client focus and the ability to establish and maintain collaborative partnerships with other health professionals and agencies.Demonstrated competence in e-health record systems and office software applications.High level written and oral communication skills.Experience working in the Aboriginal Community Controlled Health sector.Experience working in a multi-disciplinary team.Experience in delivery of group-base interventions.Experience implementing public health nutrition strategies.
