Job DescriptionKalwun Development Corporation Ltd is an Aboriginal & Torres Strait Islander community controlled organisation currently seeking to fill the following vacancy within our Health and Wellbeing Servicesdivision. Kalwun Health and Wellbeing Service delivers a primary health care service to the people of the Gold Coast & surrounding communities.
About the Role Kalwun is seeking to recruit a Social & Emotional Wellbeing Care Coordinator to provide coordination of supports to Aboriginal and Torres Strait Islander clients who are experiencing mental health and/or substance misuse conditions. We have the following positions available:
Full time at Coomera clinicFull time at the Bilinga clinic.Position Duties Triage & assessment of incoming referralsMaintain the intake register and manage wait lists effectively and timelyCollaborate with the Senior Practitioner to assist with triage of referralsUndertake assessment needs of clients and effectively manage and coordinate service delivery and provide case managementProvide brief intervention for clients presenting for mental health wellbeing and substance misuseRespond to crisis/acute presentationsProvide access and coordination to comprehensive support services for clients with the aim of achieving wellbeing as part of a holistic model of careSupport the goals of the client and that the goals are focussed on building the resilience and capacity of the clientWork with, & provide guidance for support workers to coordinate & co-manage care for clients referred to the programBachelor or Diploma in Health, Human Services, Community Services sector or relevant fieldDemonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goalsAbility to work with a culturally and socially diverse group of clients and to apply a culturally responsive framework to guide interventions, with the support of staff and managementDemonstrated skills and ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander peopleAbility to work with diverse people and presentations in providing case managed social work driven supportsDemonstrated ability to record accurate and consistent case notesDemonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative servicesWillingness to travel across the Gold Coast region and work across different sitesProficiency in using Microsoft Office and other relevant computer software or ability to rapidly acquireCurrent Qld C class drivers licence.What we offer
