Location: Sydney, NSW (Parramatta, Blacktown, Fairfield and surrounding areas)
Position: Fulltime Monday - Friday Remote with frequent travel required

The Position:
The Clinical Case Manager is responsible for managing a cohort of Home Care Package Clients including:
Manage client care plans, ensuring all services and resources are utilized to enhance client health outcomesConduct clinical assessments and provide hands-on care, along with clinical guidance for other team membersServe as the primary contact for clients, families and advocatesLead the onboarding process for new clientsOversee planning, implementation and ongoing evaluation of careEnsure all client services are delivered in line with Respect Aged Care's values, organizational policies, compliance standards, and budget requirementsThis role offers flexibility and autonomy, with opportunities to work from home and travel within the local areas.

Requirements Essential: Current AHPRA Registration Registered Nurse degreeExperience in Case Management and Aged CareHighly developed communication and relationship management skills including strong documentationExceptional problem-solving skills and ability to relate with those navigating the Aged Care systemDemonstrated organisation and prioritisation skillsAgility and flexible in styleCan do attitudeA professional attitude and presentationDesirable: Experience managing caseloads of Home Care Package Clients with complex needsExpertise in safety practices within the Aged Care or Health SectorsProficiency in conducting assessments, care planning, implementation, evaluation, and documentation of quality client care in line with Consumer Directed Care principlesStrong communication skills to liaise with staff, consumers, service providers, and health professionals, ensuring the delivery of consumer-focused servicesTo be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry.
