Social Worker | Blue Care | Telehealth & Community Care Employment Term: Full-Time Permanent contract.

A hybrid role with Telehealth and Face-to-Face Community Care options.
You will be providing critical mental health and wellness support to BlueCare's in-home Community aged care customers.
Your role will deliver high-quality, clinical support to clients, working in partnership with a collaborative, multi-disciplinary team to deliver an exceptional standard of person-centred outcomes.
You will provide our clients with Social Worker services with specialist advice and clinical services to improve their quality of life and well-being
This includes developing and implementing treatment & intervention programs, case management, clinical counselling, debriefing following critical events and education & support for clients and carers.
It is the ultimate in work/life balance - work the hours that suit you best (Monday to Friday).
You will be scheduling and managing your own case workload – so you will have flexibility to work around your personal circumstances.

Previous experience in a health or aged care context is highly desirable.
You'll also have:
Tertiary Qualifications in Social WorkRegistration with relevant professional body - Australian Association of Social WorkersPrevious experience within an Aged Care or Acute setting (desirable)Excellent interpersonal and written communication skillsExperience with technology incl.
