Our Opportunity to You We are looking for a talented Registered Nurse to join our Remedy family.
Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia.
This hybrid role will be working out of our Perth Office and from home on a Permanent full-time basis.
The Day to Day Coordinating and managing acute nursing Hospital Care at Home programs for clients discharging early from hospital to home-based care.Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment.Implementing, monitoring, and modifying in-home care plans to meet client needs.Managing clinical case escalations and variations to expected clinical progress.Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers, and funders.Helping to improve the health and wellbeing of hundreds of Australians.About You Qualification as a Registered Nurse with current AHPRA registration.Experience with complex wound care and IV antibiotic management.Experience working in a multidisciplinary collaborative team.Excellent knowledge of hospital discharge planning.Ability to work in a dynamic environment with tight timeframes and competing priorities.Excellent clinical reasoning and problem-solving skills.Excellent verbal and written communication skills.Strong administration and time management skills; tech-savvy.Passionate about health and committed to making a real difference in the community.If this role interests you, click the APPLY button to submit your application with us.
