HammondCare provides care primarily through hospitals, residential care homes and home-based care as well as services for dementia support, community palliative care, rehabilitation, positive ageing and older persons' mental health.About the roleThis role provides pastoral care, leadership and support to clients, their families, team members and pastoral care volunteers.The Pastoral Care Coordinator supports clients by visiting them in their own home across the Scone region and surrounding areas.
The role requires access to a car (mileage reimbursed) and is Permanent Part-time working 2 days per week (15.2 hours pw) either Tuesday and Wednesday or Wednesday and Thursday.
When not visiting clients the position is based at Scone.Responsibilities :Assessing client/family's spiritual and emotional needs and developing a spiritual care plan, to ensure their spiritual needs are acknowledged and documented.Providing spiritual and emotional support to clients and their families.Contributing as appropriate to carer support groups and other faith supporting activities.Liaising with relevant religious/spiritual leaders to coordinate provision of spiritual support to clients and their families where appropriate.Providing prayerful spiritual/emotional support to team members, and distributing relevant material, articles and books as appropriate.Training team members in areas related to pastoral care, including boundaries, grief and loss, faith and hope, and other areas as identified in conjunction with key stakeholders.Working with team members to extend their skills in meeting the spiritual needs of clients, including identifying suitable learning and development resources.Participating in opportunities such as staff meetings, in-services, expos, and other events with the purpose of providing information about pastoral care opportunities and creating partnerships.Regularly meeting with local service managers and team members for the purpose of receiving feedback, identifying need, and reporting back on any changes or developments in the Pastoral Care service.About youCommitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action.Strong understanding and experience of people living at home with life-limiting illness including dementia, the impact on carers, and journeys of grief and bereavement.Strong personal and active Christian faith and church involvement.Theological/Pastoral Care qualifications and experience.Demonstrated capacity to pastorally care for and enhance the wellbeing of those in need.Highly developed communication skills including listening, compassion and empathy.Experience raising up, leading and supporting a team.Strong administrative and computer skills.Access to a car, Australian driver licence and comprehensive car insurance.BenefitsWe care about your wellbeing at HammondCare and as a result offer a wide range of health and lifestyle benefits including:Fitness Passports; Full facility access can include gyms, group classes and pools for yourself and your family.Discounts across a range of household names such as JB HI-FI.Option to purchase up to 2 weeks additional annual leave.14 weeks Paid Parental Leave prorated.Learning and development opportunities.To ApplyIf you are ready to make a difference with HammondCare, Apply Now
