We also work to empower individuals, families, and communities to optimise their health.WHAT WILL YOU FOCUS ON?Provide quality care coordination for referred Danila Dilba Health Service (DDHS) clients with chronic disease and complex needs to support them in accessing relevant services according to their care plan with the aim of self-management in Primary Health Care (PHC).RESPONSIBILITIESWork in the clinic team context in undertaking a range of care coordination activities for DDHS clients with chronic conditions and complex needs in accordance with their management plan including:Working effectively with Indigenous outreach workers to engage with clients.Arranging services outlined in the client management plan (e.g. specialist and allied health appointments, arrangements for home help, connections with support groups, etc.)Ensuring transport arrangements are in place to enable the client to attend appointments.Involving the client's family / carer as appropriate.Assisting clients to adhere to treatment programs and develop chronic condition self-management skills.Assisting the client to participate in regular reviews by primary care providers.Providing appropriate clinical care consistent with the requirements of the role (this may be in the client's home or clinic setting)Maintaining quality client medical recordsEmploy a holistic, culturally appropriate approach to the client, drawing out cultural or social issues which may be impacting the client.In consultation with other team members, work to develop and implement solutions to client needs (e.g. referral to relevant service providers for support regarding accommodation, finances, well-being, and daily living, etc.)Use shared understanding of the practical realities facing the client to build a culturally sensitive health plan which increases client ownership and self-determination and communicate effectively.Expedite client access to urgent and essential allied health or specialist services, necessary transport, and GP-approved medical aids through appropriate use of the Supplementary Services Funding Pool for eligible DDHS clients.PerformanceComplete and participate in regular formal and informal performance feedback and improvement sessions.Demonstrate responsibility for own development through participation in all learning opportunities.Actively model DDHS Code of Conduct Policy and demonstrate appropriate and professional workplace behaviours, help team members as required and undertake other key responsibilities or activities as reasonably directed.Communication and TeamworkEffectively communicate with a multidisciplinary team including clients and external service providers to ensure external stakeholder services to clients.Maintain a positive work environment along with harmonious and cooperative relationships with all employees, and clients.Develop and maintain effective relationships internally and externally.Represent DDHS in a variety of settings ensuring the organisation's brand is protected and enhanced.Maintain professional work ethics and participate in staff meetings.Safe Practice and EnvironmentConduct all activities in a manner consistent with national health accreditation standards, DDHS Occupational (Work) Health and Safety Policy and procedures.Proactively address WH&S hazards, incidents, and injuries.Comply with established DDHS clinical standards, policies and protocols including (but not limited to); cold chain management; infection control; and DD Medicines Guidelines.QualityIdentify and make recommendations on opportunities to improve processes, quality, and service delivery outcomes.Participate in the development of quality procedures and contribute to internal and external program reviews as required.Implement/follow the DDHS incident and complaint investigation policies.Comply with DDHS Privacy and Confidentiality of Health Information Policy.Ensure all documentation, records management and the health information system database are maintained and completed in accordance with established standards, policies, and protocols to ensure continuity of care.Organisational ResponsibilitiesBuild and maintain awareness and understanding of the DDHS strategic plan.Contribute to developing and implementing strategies to meeting goals set in the provision of comprehensive primary health care to the Indigenous people of the Greater Darwin Area.Adhere to all organisational policies, procedures, standards, and practices e.g., Information and records management, confidentiality.Act only in ways that advance DDHS objectives, values, and reputation and with honesty, integrity, and good faith.Other duties as required, consistent with skills and experience, as directed by the Supervisor.QUALIFICATIONSQualification as a health professional (e.g., Nurse or Aboriginal and Torres Strait Islander Health Practitioner) and current registration with AHPRA. (Consideration may be given to other appropriate qualifications/ training in specific circumstances and in consultation with the Department of Health)Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and / or cardiac disease) and relevant management strategies.Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and wellbeing of Aboriginal and Torres Strait Islander people.Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.Understanding of practical realities facing clients to build a culturally sensitive health plan which increases client ownership and self-determination.Well-developed knowledge of relevant health and related services in the Darwin region (or ability to develop this knowledge).Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.High level of personal resilience and flexibility with the ability to deal with challenging situations.Demonstrated computer skills especially in the use of client information systems or similar.Experience working in a clinical practice or community health education program.Contribute to the efficient and effective functioning of their team or work unit to meet the organisation's objectives. This includes demonstrating appropriate and professional workplace behaviours in accordance with the Code of Conduct, assisting team members if required and undertaking other key responsibilities or activities as directed by one's supervisors.Read, understand, and comply with all DDHS policies and procedures.Be aware of the requirements of the DDHS's risk management framework; identifying, assessing, managing, and escalating risks; and exercising a duty of care.Work at and travel between other DDHS locations may be required from time to time.Complete all mandatory training as required by DDHS.Demonstrate understanding of the principles of anti-discrimination, staff and trainee's equity, work health and safety and other relevant legislation, and show the willingness and capacity to implement equal employment opportunity and work health and safety plans, policies, and programs.Work Health & Safety Ensure all activities comply with WHS legislation and DDHS policy & procedure.Implement & monitor risk management activities including, but not limited to, workplace inspections, incident/hazard investigation and follow-up, and implementation of risk controls.Analysis of WHS training requirements for staff including the provision of training and monitoring of training to ensure completion and currency.
