Allied Health/Health Practitioner Cadet

About the role The Social Work cadet performs a range of clinical support activities and on-the-job skills development while engaged in formal full-time tertiary studies necessary to acquire an entry-level (mandatory) qualification for employment as a Health Practitioner in Queensland Health.
The cadet will work under the delegation and supervision of a Social Worker in the provision of effective patient care as a member of the Complex Chronic Disease team.
About you Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Perform a defined range of clinical assessment and interventions as delegated by Social Worker/s and work under their supervision.Initiate changes to interventions under the supervision of Social Worker/s using results from standardised assessments, decision support tools, clinical pathways and/or patient guidelines.Provide education to patients, their families and/or carers on a defined range of topics.Communicate effectively with all members of the team regarding patient care and progress, including relevant clinical documentation.Refer to and liaise with other care providers to achieve streamlined patient care.Actively participate in professional and multidisciplinary meetings including case conferences and departmental meetings.Contribute to quality improvement and/or research projects.Health Equity It is expected that all Metro North Health staff, including the incumbent of this role as a valuable member of the Metro North workforce, contribute to the health equity agenda and meet the intent of supporting the defined six actions that specifically meet the needs of Aboriginal and Torres Strait Islander people within the National Safety and Quality Health Service Standards (NSQHS), by actively supporting the elimination of racial discrimination and institutional racism; supporting increased access to health care; influencing the social, cultural and economic determinants of health; supporting the delivery of sustainable, culturally safe and responsive health services; and recognising the importance of working with Aboriginal and Torres Strait Islander peoples, communities and organisations to design, deliver, monitor and review the health and support services we provide.
