Service Coordinator

My Support My Way is a registered NDIS service provider in South Australia and Victoria.
MSMW provides support to individuals with multiple disabilities (Mental Health/ Challenging behaviour/ emotional regulation in common).
We focus on a holistic model of care and skill development.
MSMW will work with individuals to set up tailored support when and how they would like it delivered.
We are looking for a Service Coordinator to look after business in Mount Gambier.
Job description and Responsibilities:
The Service Coordinator is responsible for supporting and managing the teams to ensure quality outcomes for clients.
Engage with clients to ensure delivery of outcomes against their service agreement with MSMW.
Work with staff to offer and deliver innovative solutions to complex problems.
Work with the CEO and COO to review and redevelop offered services.
Develop detailed work plans with reporting teams, with clear Key Performance Indicators for the delivery of outcomes to clients.
Oversee and measure outcomes for all participants supported by reporting teams.
Manage recruitment and orientation for new staff.
Contribute to the maintenance of prescribed registers, reporting systems, and client records ensuring adherence to matters of confidentiality and diversity within a sensitive environment.
Participate in organizational planning as part of the leadership team.
Keep accurate and complete records of your work activities in accordance with legislative requirements, information security, and privacy policies.
Monitor legislative and systems compliance (internal and external).
Keep accurate and complete records on the Client Management System in accordance with MSMW policies and legislative requirements.
Take reasonable care for your own health and safety and that of others in the workplaces you manage by working in accordance with legislative requirements and MSMW's Health and Safety (OH&S) policies and procedures.
Qualifications/Certifications:
Current Senior First Aid Certificate Australian Driving License and own car Current DHS (child-related) Clearance Report/ NDIS Worker Screening check A relevant qualification in the Disability/Community Services sector Demonstrated leadership experience in managing large diverse teams WAGES: As per the industry award.
Travelling allowance (Fare Work Australia) Other operational-related expenses will be reimbursed.
