About the Role
Join our growing team in Darwin on a 12 month contracted full time basis with opportunity for renewal.
Demonstrate organisational and leadership skills in the coordination of services to assist people with a disability in a community based environment.
The position is focused upon ensuring that the individual needs of clients are met, whilst complying with the NDIS framework and maintaining an efficient and safe working environment for team members.
Key Responsibilities Ensure the team complies with the Disability Services Act Standards, NDIS framework and other relevant legislationApply person-centered approach and a solution focused and strength approachCoordination and management of the day-to-day operations in our Supported Independent Living programCoordination and development of clients' care plans in liaison with families and other stakeholdersMaintaining of participants records and providing updates on progress in weekly, monthly or quarterly reportsCoordinate and oversee rostering and timesheet requirements according to client support needsProvide supervision and support to team membersProvide daily care, supervision and transport to clientsDevelop and support individual care plans, including medication, activity schedules, risk assessments, hygiene and meal requirementsProviding support to work towards individual goalsEncourage community engagement to support and increase life skill of clientsDevelopment of professional relationships with the participants, their families and other stakeholders and engagement and regular stakeholder meetingsProvision of accurate and timely case notes, incident reports and house reportingUse of Brevity Care SoftwareAssessing clients' needs and planning, developing and implementing educational, training and support programsInterviewing clients and assessing the nature and extent of difficultiesMonitoring and reporting on the progress of clientsReferring clients to agencies that can provide additional helpAssessing community need and resources for health, welfare, housing, employment, training and other facilities and serviceLiaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and servicesSupporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutionsSupervising offenders on probation and paroleAssisting young people to solve social, emotional and financial problemsPreparing submissions for funding and resources, and reports to government bodies and other agenciesSkills & Experience Cert IV or higher in Disability, Mental Health, Social Work or other relevant qualificationDemonstrated experience of working in a leadership/management role in a human services organisationDemonstrated experience in delivery of services to people with a disability in a community access and drop-in support environmentStrong coordination, problem solving and leadership skillsExperience supporting people with high support needs and personal careKnowledge of trauma and its effects on the development and behaviours of peopleThe ability to contribute positively within a teamNDIS knowledge and awarenessBasic business understanding and the importance of professional attitude and meeting deadlinesCompletion of NDIS mandatory training modulesA current First Aid CertificateAvailable to work on a Monday to Friday dayshift rosterSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, Working with Children Check, NDIS worker screening check.
