We are seeking an experienced Triage Delegate - Aged Care Assessment Services who is clinically qualified to join our team.

We specialise in assisting older adults, individuals with disabilities, those experiencing mental health challenges, and people at risk of and experiencing homelessness.
We are recruiting to deliver the Aged Care Service (ACAS).

ABOUT THE ROLE The purpose of this position is to support the delivery of the Footprints Aged Care Assessment Service (ACAS) by undertaking triage duties and determining eligibility as a delegate of the Aged Care Act (the Act).
The ACAS is designed to deliver client-centred, comprehensive, fair, and timely assessments, empowering older Australians to access Commonwealth-funded aged care services.
As a newly established service, ACAS will provide both home and comprehensive assessments targeted to meet the specific needs of senior Australians.
KEY ROLE ACCOUNTABILITIES Lead the process when triaging a referral for assessment by :Reviewing information from the registration and screening processGathering additional information from the client over the phone on their individual circumstancesDetermining as a delegate under the Act whether the client is eligible for an assessmentConfirming whether the client requires a Home Support or Comprehensive assessment and priority levelIf required, issuing an urgent referral to a service provider for time-limited supportAssigning the assessment to an appropriately qualified aged care needs assessorScheduling the initial aged care needs assessmentExercising duties as a delegate under the Act with due care, diligence, honesty, and in good faith, being mindful and accountable for decisions made under the ActEnsuring performance meets the Department of Health and Aged Care (DOHAC) contractual requirements including KPI performance that pertains to the Triage Delegate functionSupporting the reforms within Aged Care legislation, assisting clients and stakeholders to understand the new aged care needs assessment process and its role in accessing Government funded aged care services.ABOUT YOU To be successful in this role, you will need : Tertiary qualification in a health-related discipline directly related to health, aged care, or a related specialist area (e.g., registered nurses, medical officers, occupational therapists, physiotherapists, and social workers)Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or eligibility to be part of a relevant professional associationDemonstrated understanding / experience of the provision of community-based support services to the aged which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processesDemonstrated knowledge and understanding of Aged Care legislation and current reformsDemonstrated knowledge and / or experience in working with a diverse client group including people who are ageing, people experiencing mental illness, people experiencing homelessness, and people with disabilities and their carers / familiesWHY FOOTPRINTS?
