Community Support Coordinator, Regional NTCommunity Support Coordinator - Regional NT (Part-Time)Join HeartKids in making a difference We are looking for two part-time Community Support Coordinators (2-3 days per week) to support families impacted by childhood-onset heart disease (CoHD) across the Northern Territory.About the RoleHeartKids is a national not-for-profit dedicated to supporting individuals and families impacted by CoHD, including Rheumatic Heart Disease (RHD), which disproportionately affects Aboriginal and Torres Strait Islander communities in the NT. We encourage applicants with lived experience of CoHD or RHD, or a background in community support, especially Aboriginal and Torres Strait Islanders.This hybrid role is based in Darwin and at the Royal Darwin Hospital's pediatric cardiology clinic, with occasional remote work. Salary packaging is available.As a key member of our support team, you will:Provide direct support to families impacted by CoHD via in-person and digital platformsConnect families with HeartKids programs, peer mentors, and advocacy networksFacilitate coordination with health providers and social workersRepresent HeartKids at community events and run local support activitiesOccasionally travel to Katherine and Alice Springs to extend supportKey Requirements:Lived experience of CoHD and/or Cert IV in Community ServiceOpen driver's license, Working with Children Clearance, Police Clearance, and required immunizationsStrong empathy, adaptability, and communication skillsSelf-motivated, able to work independently, and committed to delivering exceptional supportYour application will include the following questions:Which of the following statements best describes your right to work in Australia?How many years' experience do you have as a Community Support Coordinator?Do you have a current Working With Children (WWC) Check?Have you completed a qualification in community services?Do you have a current Police Check (National Police Certificate) for employment?How many years' experience do you have as a Family Support Coordinator?Do you have experience working in the not-for-profit sector?Do you have a current Australian driver's licence?
