An exciting opportunity for a dedicated Enrolled nurse to join our Community Team.
Your role is to provide support, education and resources to consumers who still live in their own homes or visit our day centres.
You will support our consumers make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet their needs.
You will require experience in assessing and coordinating care and support services for consumers accessing:
Commonwealth Home Support Services and / orHome Care PackagesThis is a fixed term contract until December 2024, Monday to Friday 8:00am-16:30pm.

AHPRA Registration as an Enrolled Nurse.National Police Check (Newcastle Anglican will consider the relevance of any reported outcomes).Who we are?
