Position Profile And Role We are seeking a Clinical Incident Management Program Coordinator to lead clinical incident and risk management programs across the Child & Adolescent Health Service (CAHS), enhancing patient safety and ensuring compliance with national standards across multiple services.
Reporting to the Director Clinical Governance, Safety & Quality, you will be responsible for leading the clinical incident management program across CAHS and supporting service safety and quality teams in implementation.
You will work with directors, department heads and service safety and quality teams to implement and evaluate systems and processes that support the organisation to effectively manage and learn from clinical incidents.
You will educate and support staff in clinical incident awareness, investigation methods, and risk management principles as a key activity in embedding the CAHS clinical risk management system.
Key Responsibilities Include: Developing, implementing, maintaining and overseeing the CAHS clinical incident management program, including notification, severity assessment, and investigation of serious adverse events (SAC 1 and high-risk SAC 2).Providing expert advice to stakeholders on all aspects of clinical incident management.Supporting stakeholders in reviewing significant incidents requiring Root Cause Analysis (RCA) and other investigation methodologies, including the provision of training and resources.Monitoring, identifying, and escalating clinical incidents that pose significant risk to patients or the organisation to CAHS Executive.Implementing and maintaining the CAHS Open Disclosure framework.Coordinating external reporting to regulatory bodies.Collaborating with the Safety Quality Team on national standards and accreditation processes, ensuring open communication and promoting a culture of innovation and excellence.The ideal candidate will have extensive experience in clinical incident management and investigation methods and risk management principles, with strong analytical skills and the ability to interpret incident data and identify trends.
