The Exciting Opportunity Care Coordination: Manage Home Care Package (HCP) and private clients, ensuring individualised care aligns with budgets and legislative standards Client Advocacy: Conduct assessments, update care plans, and act as the key liaison for clients, families, and service providers Compliance & Quality: Ensure services adhere to the Aged Care Quality Standards and Commonwealth guidelines while maintaining high care delivery standards Budget Oversight: Assist clients in managing care budgets, tracking funding allocations, and recommending additional services as needed What Makes This Role Great Valued Contributions: Be part of a workplace where your efforts directly impact lives and are genuinely appreciated Team Culture: Collaborate with dedicated professionals passionate about delivering exceptional care Be guided and supported by your dedicated 3D Recruit Career Consultant, assisting with CV creation, interview preparation and career advice Salary: $90,000 pa plus super About You Experience: Minimum two years in case management, preferably in aged care, health, or home care services Skills: Strong communication, problem-solving, and organisational abilities.
Proficiency in care coordination tools is essential Knowledge: Familiarity with HCP and relevant aged care legislation.
