Located in North Adelaide, as our valued Clinical Assessment Nurse, you will fulfill a substantial number of pre-admission clinical assessments.
These assessments are crucial in forming comprehensive clinical profiles for individuals seeking residential placement, whether for respite or permanent care into our metropolitan and regional aged care homes.
This responsibility may involve intrastate travel, overnight stays, and occasional out-of-hours work.
Your role ensures that every customer receives exceptional support throughout their pre-admission journey with Helping Hand.
Your contributions will include:
Fostering a culture of exceptional customer service by delivering centralised, customer-focused care that embodies respect and fairness.
Exhibiting strong clinical knowledge and decision-making skills to deliver timely and comprehensive clinical care profiles for new residents seeking residential placement.
Collaborating to identify, optimise, and implement improved clinical assessments, ensuring a seamless customer journey across the admissions team and residential services.
Actively engaging with and promoting Helping Hand services to important external stakeholders such as community groups, referral sources, and industry representatives.
Demonstrating a continuous commitment to personal development and eagerly participating in opportunities for ongoing learning and growth.
What you'll bring To succeed in this role, you will have:
Current registration as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA).
A minimum of four years of experience, preferably in an aged care setting, resulting in a thorough understanding of, and experience in implementing contemporary assessment and care planning practices.
Highly developed computer skills, proficient in Microsoft Office, telephone systems, and customer relationship management systems.
Sound understanding of contemporary aged care requirements.
Exceptional written and verbal communication skills.
Current driver's License
Have a valid NDIS Worker Screening clearance or be willing to obtain one.
