ABOUT THE ROLEAs an Aged Care Coordinator - CHSP, your role is to manage the service for new Commonwealth Home Support Program (CHSP) clients.
You will provide excellent customer service to clients, families, and stakeholders, ensuring all inquiries are addressed promptly and professionally.
You will assist with the intake of clients and scheduling of services and take on a leadership role to a team of Home Care Assistants to deliver quality Home Care services.
This will include providing guidance, mentoring, and training for staff, providing learning opportunities and development, and supporting staff to deliver quality care that promotes client choice and independence.
WHAT WE ARE LOOKING FORYou will be passionate about providing high-quality services to our clients, have fantastic coordination skills and ideally experience within the CHSP program.
You will have the ability to communicate effectively and foster a great team environment.
Previous experience in a similar role will be highly regarded.
