The Role The Team Leader, Senior Clinician – Connections Program, is primarily responsible for the day-to-day clinical operations of the Connections Program.
The Team Leader coordinates clinical operations, manages Connections Transition Coordinators' caseloads, and clinically supports the multidisciplinary team while maintaining a manageable clinical case load of people with Drug and Alcohol (D&A) use disorder who are leaving custody and returning to the community.
This role will be required to deliver and clinically advise on the delivery of specialist D&A care, ensuring that care plans are developed with necessary supports for patients, and assertively followed up in the community and linked to appropriate health and welfare service providers post release.
The Team Leader will work in partnership across all of the Network's streams, other government and non-government organisations (NGOs) across NSW to ensure continuity of care.
