HammondCare is forming an exciting new internal Allied Health Care Team to work in Residential Care.
You will have the autonomy to work with residents to achieve the best care outcomes and be part of a broader team of in-house Allied Health professionals supporting frontline teams.
Together you will deliver resident-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and Exercise & Sports Science Australia.
Essence of the role Provision of timely, effective and high-quality exercise physiologist services to residents and appropriate support to their representatives and the HammondCare team to meet individual needs and preferences.Assess residents' physical wellbeing and tailor exercise-based interventions to restore optimal physical function, health and wellness, focusing on enabling choice and the best outcomes for the resident.Collaborate closely with residents, their representatives, staff, and the broader Allied Health Team to assess functional care needs, set care goals, and provide treatment and interventions for the best possible outcomes.Participate as a member of the HammondCare team in applying the Mission and values to pursue its objectives in a person-centred, continuous quality improvement work environment.Engage in quality improvement projects and continuing professional development.About you Commitment to engage with HammondCare's Christian Mission, Motivation, and Mission in Action.A tertiary qualification in Exercise Physiology and accreditation as an Exercise Physiologist with ESSA.Practical experience or sound knowledge in Aged Care.A current driver's license, safe driving record, and access to your own car, with a willingness to travel between HammondCare's sites and services.Effective communication and interpersonal skills (written and verbal).Demonstrated organisation and time management skills.Ability to work independently and as part of a multidisciplinary team.About HammondCare HammondCare is an independent Christian charity serving families in Australia since 1932.
