Home Support Assessor - Aged Care Assessment TeamMackay Hospital and Health Service provides health services in a range of regional, community and rural settings.

Our workplace is fast-paced, challenging, and supportive, where every employee plays a critical role in our success.Your opportunityTo provide non-clinical home-based face-to-face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT).
Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.About the TeamThe Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community.
The assessment system aims to simplify and improve access to aged care services.
