Care Manager

$50.20 per hour Casual Your Role You'll be responsible for planning, managing and leading client care and support across a range of programs.
You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.
As a Care Manager you will: Manage and coordinate care services to ensure the highest standards of careDevelop and implement care plans tailored to individual client needsLiaise with clients, families, and healthcare professionals to ensure comprehensive careFoster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goalsEnsure compliance with all relevant legislation and organizational policiesMonitor and evaluate care services to continuously improve quality and efficiencyMonitor the budget of Home Care Packages and ensure clients understand their funding and feesHow You'll Benefit You'll become part of the Ozcare family, and in addition to the training and support that you would expect, you will also receive:
Optional salary packaging, meal entertainment cards, and novated leasesPrivate health insurance discountsUniform and laundry allowanceEmployee Assistance ProgramFor Success you need to: Demonstrate experience in case management and client care coordination in a Community Aged Care settingHave demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of peopleProvide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)Hold a current Police Screening CertificateProvide a copy of your current First Aid and CPR CertificateHold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)Hold a current Queensland driver's licenceCompleted tertiary qualifications and/or relevant experience in a health-related field (e.g.
