Looking for a new approach to provide patient care?The Role:An exciting and rewarding opportunity exists for a dedicated and passionate Registered Nurse, Occupational Therapist, or Physiotherapist to join Northern Health as a Patient Watch Health Coach.Patient Watch is a service that aims to better meet the needs of some of our most vulnerable patients, especially those with complex and chronic health issues.
This innovative service will deliver more health care at home with the potential to reduce the need to visit the Emergency Department and to be admitted to hospital.
Patient Watch is based at Bundoora Centre.This is a temporary part-time position based at Bundoora Centre.The Health Coach role supports vulnerable patients to remain healthy at home.
Responsibilities include:Build relationships with patients and their families to better understand and address potentially complex factors affecting health behaviour.Manage signs of health decline, linking with health service providers and applying clinical management of patients within the scope of practice.Contribute to service improvement and development within a team environment.To be successful in this role, you will have:At least 7 years of clinical experience in the relevant profession, with at least 4 years concentrated in the care of patients with chronic and complex health needs in the public sector.Strong assessment and critical thinking skills with a multi-dimensional approach to addressing complex needs of vulnerable patients.A willingness to learn and an ability to adapt in delivering a novel and evolving service.Proven ability to build relationships and collaborate with patients and colleagues.Next Steps:If this opportunity sounds like the next step in your career, then we would love to hear from you.
