With the office based in Northbridge, YouthBeat provides support to young people that are present in the Perth inner city and Northbridge areas, that are either homeless or at risk of experiencing homelessness.
YouthBeat provides a safe supportive place for young people to wait while arranging their return to safe accommodation.
Young people are engaged through street-based assertive outreach and through the Youth At Risk Strategy (YARS).
The service is well known across Perth and has received recognition in the press for the support provided to street-present young people in the Perth inner city and Northbridge areas.
As a YouthBeat Case Manager, you will play a vital role in providing case coordination that empowers young people and their families/carers to overcome challenges and achieve positive outcomes.
This includes offering outreach case coordination, counselling, and support to address risk factors impacting their lives.
Your Key Responsibilities Work with young people accessing the service to create holistic and strength-based individualised case/support plans using SMART goals, including referral to supplementary services as needed.Respond to referrals to the service from internal and external support services and coordinate case allocations.Develop strong professional relationships with young people and/or their families, key stakeholders, and staff to contribute to the effective functioning of the service and improved outcomes.Work collaboratively with service staff to provide specialised support to address complex cases or emergency situations.Balance the desire to support the young people, whilst maintaining a focus on achieving the required performance and contractual outcomes.Provide on-call support for staff across different services.Required to work one Friday night YARS shift roughly once a month.Requirements for Success A Degree or Diploma in Social Welfare/Social Science/Youth Work or a minimum of three years' experience in a similar role.Demonstrated experience providing case coordination, assessment, counselling, and crisis intervention, including with people experiencing alcohol and other drug use issues, mental health issues, personal crisis, and families in crisis.Experience and ability to work with at-risk young people, including knowledge of child and adolescent development and child and youth safe practices.Demonstrated experience working with Aboriginal and Torres Strait Islander communities.Computer skills in Microsoft Office, and ability to adopt Mission Australia's web-based Information Management system (training will be provided).Full driver licence with no restrictions.Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
