About Us Home Caring is a private home care company that provides aged care and disability support across South Australia.

With the mission of proudly enriching lives with genuine care, together, we are excited to welcome you as our next Case Manager - Disability Services.
We are committed to delivering sensitive and respectful client-centred care by offering a wide range of services such as personal care, meal preparations, domestic assistance, transport, and social support in the comfort and privacy of the clients' homes.
Key Responsibilities Conduct comprehensive assessments of clients' needs to facilitate coordination of supports and aligning with clients' NDIS goals, considering physical, emotional, and social factors.Develop individualized support schedules that facilitate our clients to achieve their goals in collaboration with clients, their families, and healthcare professionals.Act as a central point of contact, coordinating and liaising with clients, their families, healthcare providers, social services, and community resources.Foster effective communication among interdisciplinary teams including our support workers to ensure holistic and seamless care delivery.Maintain accurate and up-to-date client records, documenting assessments, reviews, and progress notes.Prepare detailed reports for internal and external stakeholders as required.Advocate for clients' rights and needs within the healthcare system.Provide emotional support and guidance to clients and their families throughout the disability support delivery process.You will be supported by a collaborative team in our fast-paced environment, supporting and driving meaningful disability service delivery, as well as liaising with clients and our partners and stakeholders.
You will be a key part of our team in a growing organisation.
To successfully fill this role, you will have: Proven experience in case management or care coordination within disability services is a must.Tertiary qualifications in Nursing, Social Work, Psychology, or a related field is highly desirable.Outstanding interpersonal and communication skills.Ability to work collaboratively in a team and independently.Empathy, compassion, and a commitment to upholding the highest standards of care.Exceptional problem-solving and time management skills thriving in a fast-paced environment.Ability to establish respectful relationships with clients and support workers.Strong attention to detail and a high level of proficiency in written and verbal communication skills.Microsoft Suite expertise is preferred.Next Step As an organisation, we are committed to protecting children, young people, and vulnerable adults.
