Be part of something bigger. We are seeking a dedicated SEWB Case Manager to deliver high-quality, culturally sensitive wellbeing services to clients across East Arnhem Land living with mental illness.
Using a trauma-informed, strengths-based approach, you will provide case management, develop recovery-oriented case plans, and coordinate with stakeholders to support clients in achieving their goals.
Your role will include conducting intake assessments, organizing referrals, and delivering culturally appropriate community activities to enhance mental health and wellbeing.
Regular travel to remote communities by light aircraft is required.
This position also involves mentoring Community Workers and fostering collaboration to meet program objectives.
This is a full-time position starting in Dec 2024, based in either Darwin or Nhulunbuy, with regular travel to community for outreach. This role can be based in community; however, housing is not provided. About Miwatj Miwatj Health is an independent Aboriginal Community-Controlled Health Service.
Established in 1992, we're committed to securing quality health care services across the East Arnhem and enabling Yol?u powered community health and wellbeing.
Miwatj provides culturally appropriate, comprehensive primary health care to over 6,000 remote Aboriginal and Torres Strait Islander residents across 3 East Arnhem wards.
With a Head Office in Nhulunbuy, Miwatj also manages a Corporate Office in Darwin, 7 fixed clinics across the East Arnhem region, and dedicated outreach teams who travel to 3 additional locations.
The ideal candidate must be passionate about supporting Aboriginal Community Control and self-determination and understands the unique challenges Yolngu people face in remote communities.
With at least two years of experience in SEWB or Community Services, you demonstrate expertise in person-centered, trauma-informed, and recovery-oriented care.
You possess excellent communication and interpersonal skills, along with reliability, persistence, and a proactive, collaborative approach.
Essential requirements include an Ochre Card, strong data management capabilities, and the ability to travel to remote communities by light aircraft for extended periods.
A Diploma in Community Services or a related field and a current First Aid certificate will be highly regarded.
