Nurse Navigator (Aboriginal and Torres Strait Islander)As the Nurse Navigator (Aboriginal and Torres Strait Islander), you will aim to facilitate a safe and efficient consumer journey, through the reduction of service fragmentation, educating and empowering individuals and coordinating consumer care.Be a part of a state-wide model of nursing care designed to provide holistic support to consumers with chronic and complex health conditions requiring an expert level of healthcare support to improve consumer outcomes and quality of life.
This is a Temporary Part Time opportunity until 27 April 2025 with a possible extension.Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community. The Central Queensland Hospital and Health Service (CQHHS) Nurse and Midwife Navigator Service provides a centralised CQ-wide model of delivery, with Nurse Navigator positions operating from community care settings, based at Gladstone, Biloela (Banana), Emerald (Central Highlands) and Rockhampton (Capricorn Coast, Mount Morgan, Woorabinda).
About the role:The Nurse Navigator – Aboriginal and Torres Strait Islander employs advanced clinical and professional practice capabilities to deliver a positive consumer centred health experience across the lifespan for consumers with complex and chronic health conditions.
The four pillars that underpin the Nurse Navigator role are: coordination of consumer centred care; creating partnerships; improving consumer outcomes; and facilitating systems improvement.
About you:Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) and a current annual practising certificate are mandatory for this position.While not mandatory, a relevant post graduate qualification, or working towards one, would be well regarded.Under Section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander community.What we offer:Salary information: Total remuneration value of up to $7,112 p.m. is comprised of:
