Varying hours per week up to full-time - Contract Until January 2026We are seeking a dedicated and skilled Clinical Nurse/ Midwife to join our Corporate Team. As the Clinical Nurse/ Midwife, the role will be responsible for coordinating access to community delivered services as required to support continuation of care, providing high-quality person-centred care and preventing unnecessary ED presentations.
Staff in this role are required to maintain contemporary knowledge about safe clinical triage, hospital avoidance strategies and community alternatives for the delivery of care.
The role will provide nursing services via Telehealth in the South Australian Virtual Care Service (SA VCS) utilizing knowledge and skills that have been consolidated by experience and/or further study.
About the role: Provide proficient, person-centred, clinical nursing care and/or individual case management to patients/clients in a defined clinical areaMonitor patient/client care plans to ensure appropriate care outcomes are achievedTake responsibility for own practice standards, activities delegated to others, and the guidance and development of less experienced staff.The SA Virtual Care Services is located at Tonsley, and staff are required to work onsite.About you: Effective communication, problem-solving, conflict resolution, and negotiation skillsAbility to prioritize workload and meet set timelines, whilst working under minimal supervisionRegistered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificateProven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards.Want To Know More?
