The Care Partner role is all about making a real difference in the lives of older people. You'll help them live safely, independently, and with dignity in their own homes by delivering rights-based, person-centred care. As a trusted point of contact and advocate, you'll work closely with clients, their families, and the internal teams to design tailored care plans, coordinate services that meet the individuals assessed needs, and actively promote wellbeing and independence. Previously known as Home Care Coordinator, this position is central to the transition to the Support at Home program. Your knowledge of Support at Home reforms and the Commonwealth Home Support Programme (CHSP) will be highly valued as you guide customers through these changes while ensuring continuity of care. If you are a Registered Nurse that is looking to step away from hands on clinical work but still want to work with people in the community and maintain your registration, this is the perfect opportunity to combine your clinical expertise with care coordination and advocacy. This is a float role within our Home Care team, providing coverage and support across multiple service streams, including client onboarding. You'll travel across metro areas as needed, ensuring a seamless experience for clients and continuity of care across the program. You’ll also make an impact by: Establishing genuine, respectful partnerships with customers and their supporters, recognising and upholding their inherent rights to autonomy, dignity, respect, and self-determination in all aspects of their care. Facilitating or overseeing comprehensive assessments in collaboration with the customer, identifying their strengths, needs, goals, preferences, and cultural background while supporting their right to active participation. Working closely with customers to plan and manage their individual Support at Home budgets, ensuring alignment with their support plan and classification and identified needs. Diligently monitoring spending to maximise the impact of individual funding, preventing both underutilisation and over‑expenditure, and adjusting resource allocation responsively to evolving needs. Coordinating a range of services and supports in a way that respects customers' choices and promotes seamless transitions across different service streams. Regularly monitoring customers' wellbeing and the effectiveness of their support plans, conducting reviews in partnership with customers to ensure their evolving needs and rights are consistently upheld. Providing customers and their supporters with clear, accessible information and education about their care options, rights, and relevant community resources. Working collaboratively with clinically qualified Care Partners to ensure participants with complex needs receive appropriate clinical input and coordinated care that respects their rights. What you’ll bring To succeed in this role, you will have: Bachelor qualification in Nursing or a related health discipline is highly regarded. Strong knowledge of Support at Home reforms and CHSP. Demonstrated experience in providing care management or case management services, with a strong understanding and application of a rights-based approach. Exceptional assessment, support planning, and coordination skills, with a demonstrated ability to empower participants in decision‑making. Sound understanding of person‑centred and rights-based approach. Demonstrated commitment to professional development. Current driver’s licence and reliable insured and registered vehicle. Senior First Aid and CPR certificates or willingness to obtain them. Valid NDIS Worker Screening clearance or willingness to obtain one. Why Helping Hand?
