Murray Primary Health Network – Bendigo VIC The Opportunity The Coordinator Complex and Integrated Care will play a key role enabling the provision of person centred, trauma informed and culturally safe integrated care for people in underserved communities. Your primary focus will be on commissioning, workforce development and system integration activities that meet the goals of the Care finder program. This position offers the opportunity to contribute to impactful programs in a supportive and forward-thinking environment. Key Accountabilities Lead the coordination and delivery of integrated care initiatives, primarily for the Care finder program. Support workforce development through training, stakeholder engagement, and capacity‑building activities. Form and lead engaging Communities of Practice that build understanding of reform complexities, taking a collaborative approach to achieving program goals. Use data and evaluation practices to drive quality improvement and inform strategic decisions. Deliver meaningful outcomes for our diverse communities by leveraging catchment‑wide networks and your ability to build strong connections. Work with your team on a range of programs and projects, contributing to a culture of knowledge sharing and collaboration. About you You’re a strategic thinker with a passion for improving outcomes for older people and underserved communities. A natural advocate who thrives in complexity, brings curiosity to reform challenges, you will be energised by collaboration and continuous learning. Key Capabilities Demonstrated experience in coordinating and delivering health or aged care programs, including planning, implementation, and reporting against service milestones in a regional or rural context. Proven ability to build and maintain effective relationships with internal and external stakeholders to achieve integrated service delivery and collaborative outcomes. Strong project management skills, including the application of quality improvement and risk management principles to drive program performance. Ability to use data and evidence to inform decision‑making, evaluate program effectiveness, and contribute to strategic planning and reporting. High‑level communication skills, with the ability to engage diverse stakeholders, facilitate communities of practice, and support workforce development initiatives. Understanding of aged care reforms and the complex needs of older people, with a commitment to culturally responsive and inclusive practice.
