This position providescustomer-based services andinvolvesthe admission and assessment of participants in line with their needs and goals. By partnering and working with participants we assist in enabling them to safely live in their own home and community. The successfulapplicant will be someone who can independently manage and grow a portfolio of Support at Home clients. Key Responsibilities include: Partnering with participants to develop care plans for Support at Home Ongoing case management of existing Home Care Package consumers Proactively promote White Oak to potential participants and community referral partners (GPs, Social Workers, Assessment agencies, hospitals, Pharmacies etc.) Regular monitoring and monthly reporting on participantbudgets Working closely with White Oak Clinical and Allied teams Working with other program and rostering managers to ensure a consistently high level of service is delivered Keeping up-to-date with government legislated standards, guidelines and reforms To be successful in the role, you will be a well-presented, highly empathetic and compassionate person who prides themselves on delivering exceptional customer service. In addition, you will ideally possess: Previous experience in Home Care Package Coordination Highly developed communication and organisational skills Ability to work in a dynamic, fast-paced and changing environment High level computer literacy (MS Office applications and CMS; preferably Alayacare) Kind, caring and respectful of consumers and colleagues identity, culture and diversity.
