Case Manager – Young Women’s Support Base pay range : A$45.92/hr – A$49.41/hr. About the Opportunity The primary purpose of this case manager position is to assist with the immediate and ongoing support needs of young women or those that identify as female (16‑25), who are at risk of or experiencing homelessness, predominantly in the Southeast of Melbourne. Role responsibilities include providing a range of flexible support services and high‑level coordination to clients, using a strength‑based and trauma‑informed case management model to maximise independence and minimise service dependence. What You’ll Be Doing Conducting comprehensive assessments of clients' needs to develop collaborative case plans. Approaching and building a comprehensive network of external service providers across metropolitan and surrounding suburbs of Melbourne to receive and complete direct referrals. Providing comprehensive case management services to clients, including assistance with accommodation, advocacy, assessment, referral, and community supports. Facilitating clients' involvement in community and social activities, recreational and educational or training opportunities. Advocating on behalf of clients where agreed, liaising with mental health, drug and alcohol services, legal institutes, government and non‑government providers. Initiating and attending case management meetings with professionals involved with clients to ensure collaborative service delivery. What We’re Looking For (Key Selection Criteria) Demonstrated experience in client‑facing human services, social work or other relevant discipline, with a preference for understanding of youth housing and homelessness policy, context and systems. Tertiary qualifications in Social Work, Psychology or a related discipline highly desirable. Experience supporting people with complex challenges and support needs. Understanding of trauma‑informed practice and the individual and structural causes of addiction and dependence. High degree of initiative and analytical skills, well‑developed organisational and time‑management skills. Demonstrated capability in verbal and written communication and excellent interpersonal skills. Capacity to maintain confidentiality, complete tasks and manage time effectively. Ability to work collaboratively with other organisations to achieve client outcomes. A valid Victorian driver’s licence (mandatory). Understanding of, or commitment to, upholding Child Safe Standards in the workplace. Strong communication and interpersonal skills with ability to engage effectively with diverse backgrounds.
