Homelessness Health Workers - Multiple Positions $68,000 – $76,000 pa + salary packaging + super Ready to Make a Real Difference in 2026? We're seeking Expressions of Interest for the following rewarding roles, based on Wiradjuri Country : These roles are part of our Homelessness Health Support Model of Care , designed to improve access to primary health care for those most in need. You'll work directly and compassionately with individuals identified as homeless or at risk, helping them connect with essential health and community services. What You'll Do Provide outreach support, assess primary health care needs, and guide clients through established pathways for positive health outcomes. Create opportunities for clients to engage in community life and build social connections. Report on program outcomes and contribute to service improvements. Ensure cultural safety for Aboriginal service users. Deliver person-centred, trauma-informed care that meets the unique needs of each individual.
