About the Role Provide advanced nursing assessment and acute clinical care directly to residents in Residential Aged Care Facilities, reducing avoidable hospital presentations. Work autonomously and collaboratively with GPs, Geriatricians, and community services to deliver timely, person-centred care. Triage referrals, conduct face-to-face assessments, and support early intervention strategies within a dynamic, consultative nursing role. Maintain clear and timely communication with key stakeholders through telehealth, EMR, and written documentation to support integrated care planning. Liaise closely with residents and their families to ensure holistic, person-centred care that aligns with individual values, preferences, and goals of care. Essential Current, unrestricted Victorian Driver’s License. Post graduate qualifications in critical care. Minimum 5 years’ experience post registration. Highly developed assessment, clinical reasoning, problem solving and prioritisation skills. Strong interpersonal and communication skills both written and verbal. High level of reliability and professional conduct. As of 1 January 2026, all new employees will require a valid Victorian Working With Children Check.
