The CCLN is an integral part of the Critical Care Liaison Service which is based in the ICU. Each CCLN actively contributes to the Tracheostomy Review Service and the Medical Emergency Team (MET). Critical Care Liaison Service – Seven Basic Aims Review all patients discharged from the Intensive Care Unit Provide consultation to ward staff regarding treatment and management of patients Aid timely review and care planning of critically ill patients in the Emergency Department (ED) Share clinical skills to enhance knowledge of ward staff Avert re-admissions to the ICU or facilitate early and appropriate admission Improve communication between ICU and the acute wards Support the operational needs of the ICU in conjunction with the ICU Associate Nurse Unit Manager (ANUM) This challenging yet very rewarding role within the hospital will enable you to share your clinical experience and knowledge. As the Critical Care Liaison Nurse you will be part of the Medical Emergency Team Response; responsible for the comprehensive follow-up review and assessment of patients discharged from the ICU; facilitating timely admission / re-admission to the ICU; sharing your critical care skills with ward based multidisciplinary staff and contributing to the development and improvement of critical care services within St Vincents. This role is offered as a permanent night duty position on the basis of 10 hour shifts. Classification for this position is ZJ4 (Grade CAPR 3.2) ($ per annum). It accommodates elective and emergency surgical and medical patients. The ICU admits deteriorating patients from the ward environment Operating Theatre Emergency Department plus external admissions from other metropolitan and rural hospitals via Adult Retrieval Victoria (ARV). The ICU maintains the Medical Emergency Team Critical Care Liaison Nurse (CCLN) Team and Tracheostomy Review Service. The Department is a tertiary referral centre for Neurosurgery and Cardiothoracic surgery and is a Level 3 VECMOS Centre. Duties and Responsibilities Acts as a consultant for nursing staff and members of the multidisciplinary team in critical care nursing activities Works collaboratively with the ICU multidisciplinary team to provide best possible patient care both within the ICU and across SVHM Implements a continuous quality improvement programme in which patient care outcomes are regularly evaluated and enhanced including but not limited to streamlining the discharge process ensuring effective handover reinforcing tracheostomy education early detection of deteriorating patients and streamlining Goals of Care discussions for patients Promotes effective relationships with ICU and ward nursing staff and the broader multidisciplinary teams Contributes to the development and implementation of strategies that facilitate service efficiency and patient access Identifies patient care and needs in accordance with critical care services provided by the hospital and following hospital policy guidelines and procedures Actively collaborates with the multidisciplinary ICU team in the clinical decision making regarding the discharge of patients from the ICU including the coordination of discharge planning follow-up review of patients and consultation with acute ward staff Actively participates in Tracheostomy Review Service including daily assessment of all patients with a tracheostomy insitu bi-weekly Hospital Tracheostomy Service ward round and evaluation of service provided Actively manages the Critical Care Liaison review service including patient and family support documentation of clinical assessments referral and communication with home unit staff Facilitates the admission / re-admission of patients to the ICU if required in conjunction with ICU and home unit staff Responds to all inpatient Medical Emergency Team MET and Code Blue calls and supports the coordination and management of the response In conjunction with the ICU reviews and implements opportunities for improving services to high risk patients post discharge including but not limited to establishing new services and creating educational initiatives.
