The Department of Health plays a critical role in the Victorian health system and is responsible for shaping it to meet the health needs of all Victorians. About the role: The Principal Program Adviser will lead the delivery of the Victorian Community Health Seniors Program, which is funded under the Commonwealth's Strengthening Medicare – Supporting Older Australians (Victoria) initiative. The program is a pilot early intervention program targeted to Victorians aged 65 years and over to reduce frailty and decrease avoidable acute admissions, with a focus on improving participants' functional capacity, enhancing wellbeing, and promoting independence. This position will manage a small team and is responsible for overseeing all aspects of program implementation and providing high-quality policy and program advice to support effective design, delivery and evaluation. It requires well developed coordination skills to manage complex issues and risks and to develop and lead solutions to address them. The role involves close collaboration with internal and external stakeholders including the Commonwealth and community health services sector. It contributes to the development and implementation of innovative strategies and reforms to strengthen the community health sector and to reduce pressure on the acute health system. Knowledge and understanding of the primary and community health, and aged care sectors.
