We are seeking an organised, compassionate and proactive Home Support & Aged Care Assessor to join our team at the Lemongrove Campus. In this role, you will support the smooth delivery of aged-care services and help ensure older community members receive the high-quality assistance they deserve. This role will see you support the smooth operation of Home Support services and conduct assessments that ensure clients receive the right assistance at the right time. You will work closely with clients, families, service providers and internal teams to coordinate care, maintain accurate records and uphold aged-care quality standards. As an Aged Care Assessor, this will see you prepare clients for assessment by uploading NSAF documentation, verifying identification for clients and/or representatives, and gathering relevant background information, conduct client screening and assessments in accordance with RAS Guidelines for Home Support Assessors whilst working collaboratively with clients and/or their representatives to develop tailored Support Plans, using the My Aged Care Service Finder to match services to individual needs. Duties will include referrals through the My Aged Care Gateway and ensure all records are accurately updated and finalised upon completion of assessments, respond to referrals within designated timeframes across high, medium and low priority levels and deliver a person-centred, flexible and responsive service, grounded in wellness and reablement principles and aligned with current Aged Care Quality Standards. Statement of Attainment 2 and Self -Paced Learning Experiences x 3 (SPLEs) or willingness to undertake when commencing the role. Well-developed written, verbal and interpersonal communication skills including accurate spelling, good verbal skills and comprehension and ability to deal courteously with people at all levels. Demonstrated computer skills with capacity to learn and use electronic reporting systems and registration system. Demonstrated ability to work independently as well as effectively in a collaborative team environment. Ability to follow guidelines relevant to position as well as demonstrated problem solving capacity. Current Driver's Licence and willingness to use for work purpose travel.
