Lemongrove Campus is a leading aged‑care and community support hub dedicated to delivering high‑quality, person‑centred services for older people. We are seeking an organised, compassionate and proactive Home Support & Aged Care Assessor to join our team at the Lemongrove Campus. In this role, you will support the smooth delivery of aged‑care services, help ensure older community members receive the high‑quality assistance they deserve, and coordinate care, maintain accurate records and uphold aged‑care quality standards. Responsibilities Prepare clients for assessment by uploading NSAF documentation, verifying identification, and gathering relevant background information. Conduct client screening and assessments in accordance with RAS Guidelines for Home Support Assessors. Develop tailored Support Plans and use the My Aged Care Service Finder to match services to individual needs. Refer via the My Aged Care Gateway and ensure all records are accurately updated and finalised. Respond to referrals within designated timeframes across high, medium, and low priority levels. Deliver a person‑centred, flexible and responsive service grounded in wellness and reablement principles. Qualifications Certificate III in Home Care or Aged Care or equivalent work experience. Statement of Attainment 2 and Self‑Paced Learning Experiences (SPLEs) or willingness to undertake when commencing the role. Well‑developed written, verbal and interpersonal communication skills. Demonstrated computer skills and ability to use electronic reporting systems. Ability to work independently as well as effectively in a collaborative team environment. Ability to follow guidelines relevant to the position and demonstrate problem‑solving capacity. Current Driver's Licence and willingness to use for work‑purpose travel.
