Coordinator- Aged Care – Momentum Collective This role involves coordinating services for Commonwealth Home Support Program (CHSP) clients, providing excellent customer service to clients, families, and stakeholders, assisting with client intake and service scheduling, and leading a team of Home Care Assistants to deliver high‑quality care that promotes client choice and independence. Base Pay Range A$44.00/hr – A$55.00/hr This range is provided by Momentum Collective. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. About the Role As an Aged Care Coordinator – CHSP, your role is to coordinate the service for CHSP clients, ensuring all inquiries are addressed promptly and professionally. You will assist with the intake of clients and scheduling of services and take on a leadership role to a team of Home Care Assistants to deliver quality Home Care services. This includes providing guidance, mentoring, training for staff, and supporting staff to deliver care that promotes client choice and independence. Excellent communication, team‑building, and past experience in a similar role are highly regarded. This is a full‑time fixed‑term role to 30/06/2027. Current unencumbered Driver’s Licence. Current first aid and CPR. Satisfactory Criminal Record Check. Ability to mentor, share knowledge and information with less experienced team members. Ability to manage a complex workload by way of planning and organising priorities.
