About The Role Grampians Health Cancer Services is seeking to recruit a dedicated and enthusiastic Registered Nurse (Division 1)/ Nurse Practitioner as the Regional Renal Nurse Coordinator/ Nurse Practitioner (RRC/ RRNP), to be based at the Wimmera Cancer Centre, Horsham within the Renal Services team. As the successful candidate, you will be a renal/kidney/dialysis specialist. What You’ll Be Doing The role of the Regional Renal Nurse Coordinator/ Nurse Practitioner (RRC/NP) is to provide specialist nursing care, information, education and support to patients diagnosed with end stage kidney disease in the Wimmera region. The RRC/NP is a highly skilled and motivated practitioner who will utilise expert skills, knowledge and experience to work as a member of the multidisciplinary health care team according to the complexity of care required and the individual need of the patients in their care. Workload will be identified by the wider renal services team and this will be achieved through extensive interdisciplinary collaboration across primary, secondary and tertiary health. The RRC/NP is a clinical expert in end stage renal disease and functions as a resource person to patients, carers, families, clinical services, and community agencies. The RRC/NP will deliver a coordinated, evidenced-based, seamless, and patient focused approach to care through specialist educational resources for patients, carers, and staff. The patient-centred, holistic nursing care provided by the RRC/NP is consistent with the approach underpinned by the Grampians Health Renal Service. Essential Criteria Post Graduate qualifications in Renal/ Kidney disease. AHPRA Nursing Registration Division 1 with endorsement as Nurse Practitioner (or working towards). Minimum of 5 years full time equivalent post registration experience as a registered nurse with at least 3 years recent full time equivalent experience in Renal Services/ Haemodialysis. Selection Criteria Demonstrated ability to assess, plan, initiate and manage complex cases at an advanced level. Demonstrated understanding of the physical, and psychological needs of patients with end stage renal disease. Proven ability to implement change and develop direction in clinical practice by the use of evidence-based research and best practice principles. Excellent interpersonal oral and written communication skills and the ability to provide information and education effectively to patients, families, and carers. Demonstrated effective organisational and time management skills and the capacity to successfully manage competing priorities and meet deadlines. As the successful applicant you will be an outstanding leader and clinician. You will demonstrate an exceptional track record as a dialysis/ renal/ kidney registered nurse. You will be a confident communicator, possess and provide high-level knowledge of oncology services where evidence based clinical approaches are fundamental to the delivery of services. You will be an advocate for multidisciplinary care, and an active leader and contributor to the team approach across allied health services provided within cancer services. You will have a strong understanding of renal/ kidney systems and strategy, particularly how services are applied in regional locations. This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2024 - 2028. The classification is Clinical Nurse Consultant (ZF4-ZJ4) which ranges from $59.79 - $62.94 per hour.
