The opportunity We’re excited to offer a rewarding opportunity at Diversicare , part of the Ethnic Communities Council of Queensland (ECCQ) , for an experienced Care Partner to support our Home Care Package (HCP) clients. This full-time role is based on our Brisbane office (Indooroopilly) , easily accessible via public transport and close to local amenities. As a Care Partner, you’ll play a key role in empowering clients to live independently and with dignity in their own homes. You’ll be part of a passionate team of over 120 staff committed to culturally inclusive, person-centred care. What We’re Looking For Proven experience in Home Care services (HCP) Strong interpersonal and coordination skills Solid understanding of Aged Care and Community Care Quality Standards Strong verbal and written communication skills , with the ability to influence and build rapport A proactive mindset with problem-solving skills and keen attention to detail Cultural awareness and sensitivity in a diverse workplace environment A customer-focused approach that puts client wellbeing at the centre of everything you do Key Responsibilities Build strong relationships with clients and their families through regular check‑ins to ensure satisfaction and address any concerns Conduct comprehensive in‑home assessments and develop tailored care plans that support independence and wellbeing Collaborate with internal teams and external providers to ensure seamless, culturally responsive service delivery Monitor client needs and adjust services proactively to reflect changes in health or circumstances Maintain accurate and up-to-date client records in line with compliance and quality standards Generate regular reports to support management oversight and continuous improvement Manage Home Care Package (HCP) budgets to align with client goals and care priorities Support clients transitioning into care packages and assist with smooth transitions into residential care when needed Applications must be accompanied by a cover letter addressing the applicant’s suitability for the role.
