Join Our Team as a Health Care Navigator Are you ready to make a meaningful impact on individuals’ health and well-being? We’re looking for a dedicated Health Care Navigator to join our team in Wagga Wagga (Wiradjuri Country) on a part time (30 hours per week) basis In this role you will collaborate with our care coordinators in addressing the diverse social needs that influence health outcomes. You will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation. Selection Criteria: The Essentials for Our Health Care Navigator role Qualifications that Shine : You hold recognised credentials in Community Services, Human Services, Social Work, or a Health-related field. Advocacy Champion : Hands‑on experience advocating for and connecting participants with local services (aged care, NDIS, community services) to help them hit their health and wellbeing goals. Person‑Centred Pro : Expertise in person‑centred care and collaboration with participants and their families to create tailored support plans. Health Detective : Keen understanding of the wider determinants of health—social, economic, and environmental factors—and how they impact communities. Relationship Builder : Strong networking skills and the ability to forge partnerships across diverse organisations. Communication Pro : Excellent communication skills, capable of engaging with culturally and linguistically diverse backgrounds and First Nations communities. Tech‑Savvy : Comfortable with computers and quick to adapt to new software applications. Lifelong Learner : Dedicated to professional growth and constantly seeking opportunities to expand knowledge and skills.
