You’ll collaborate with families, carers, and service providers to make sure every recommendation is thoughtful, safe, and aligned with the customer’s goals. Whether it’s identifying support options, or linking people to meaningful services, you’ll help make aged care feel less overwhelming and more like a genuine support. The role is offered as a permanent, full-time opportunity (Monday to Friday) with travel in the Brisbane North region required in meeting with customers and stakeholders. You’ll have the flexibility to complete admin tasks from home or a co-working space. You’ll thrive in this role if you Hold a tertiary qualification in a health-related discipline (e.g. nursing, OT, physio, social work) and current AHPRA registration or eligible to be part of a relevant professional association. Communicate with warmth, professionalism, and clarity, especially when supporting people with complex health needs or sensory and cognitive challenges. Understand the impact of ageing on individuals and families Are confident conducting assessments and navigating risk with sensitivity and care Are comfortable using technology and open to learning new systems Work well independently but thrive in a collaborative team Have a valid Driver’s Licence, reliable vehicle, and current First Aid, CPR, and National Criminal History Check (or are willing to obtain them), and can meet all necessary checks and declarations required for the role.
