What The Role Looks Like Your days will take you into the community, supporting Aboriginal people across Cessnock, Muswellbrook, Singleton and Maitland. You’ll be the expert nurse who brings chronic care out of the hospital and into people’s lives. Expect a mix of clinical assessments, case management, care coordination, and working closely with local GPs and primary health networks to keep care flowing smoothly. No two days are the same. One morning you might be reviewing a care plan with a patient recently discharged from hospital. In the afternoon you might be chatting with a GP about a complex case or linking someone with the services that help them stay on track. It’s meaningful work with a personal touch, supporting mob to manage chronic disease in a way that’s culturally respectful, practical, and genuinely empowering. Requirements Registered Nurse with current Authority to Practice with AHPRA 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in this specialty field. In addition the employee must have approved postgraduate nursing qualifications in chronic disease management.
